You will need the following information about all DLC attendees:
First & Last Name
Gender (for rooming purposes)
T-Shirt Size
Office Held (if applicable, for Key Club Members)
Key Club Name (for Key Club Members)
Kiwanis Club Name (if applicable, for adult Kiwanis Members)
Key Club Division
Dietary Restrictions/Food Allergies
STEP 2: Please complete the form below. This must be completed in one session, as you cannot save and come back to it. You will need to upload the completed Excel Sheet from Step 1, and submit payment if you are paying by credit or debit card.
